Altru Rehabilitation Hospital seeks a compassionate Case Manager to coordinate care for inpatient and outpatient rehab patients in our Healthcare & Medical Services department. The Case Manager will manage caseloads from admission through discharge, complete assessments, develop individualized care plans, facilitate interdisciplinary team collaboration, and secure post-acute services and community resources to support long-term recovery. Manage a caseload of rehab patients from admission through discharge Develop, implement, and update individualized care plans Coordinate interdisciplinary team meetings and services Conduct psychosocial assessments and discharge planning Educate patients and families on resources and recovery goals Collaborate with physicians, therapists, and nursing staff Advocate for patients’ needs and support community reintegration Document assessments, plans, and outcomes in the EMR Arrange post-acute services, equipment, and home supports Participate in quality improvement and professional development Case management Discharge planning Rehabilitation care coordination Psychosocial assessment Electronic Medical Records (EMR) documentation Utilization review and insurance coordination Motivational interviewing Community resource navigation Interdisciplinary team collaboration Patient and family education Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active RN, LSW, or related clinical license (state-appropriate) Certified Case Manager (CCM) preferred Basic Life Support (BLS) certification Experience in inpatient or outpatient rehabilitation setting preferred5c143e31-5e48-4549-b638-05792d185386
