Family Medicine Physician $250K $260K + Bonuses 4-Day Option No Weekends

365 Healthcare is recruiting a Board Certified Family Medicine Physician for a full-time permanent outpatient opportunity in Stillwater, New York.

Position Snapshot
- Base Salary: $250,000 $260,000 + productivity incentives
- Sign-On Bonus: Up to $75,000 (paid first paycheck)
- Retention Bonus: $25,000 (Year 2)
- Schedule: 4- or 5-day work week
- Hours: 40 hours/week
- Employment Type: Full-time, permanent
Schedule & Work-Life Balance
- Monday Friday schedule
- Flexible 4- or 5-day work week
- No in-office weekends
- Call: Approximately 1 in every 70 days
- Phone call only
- RN triage handles first contact
- Telemedicine coverage:
- 2 3 hours on a weekend once every 3 months
This is a low-burnout outpatient model designed for long-term sustainability.
Practice Environment
- Established outpatient primary care clinic

Clinical Responsibilities
- Provide comprehensive primary care (pediatrics through geriatrics)
- Manage chronic and acute conditions
- Perform preventive care, screenings, and immunizations
- Deliver patient-centered, evidence-based care
- Collaborate with APPs and multidisciplinary teams
- Maintain accurate documentation in Epic
Qualifications
- Board Certified in Family Medicine
(or dual Board Certified IM/Peds) - Active New York license (or ability to obtain)
- Minimum 1 year of primary care experience required
- DEA and ACLS certification (by start date)
- Clean malpractice and license history
