Salary: $80,000 - $90,000

Job Summary

Provide emotional, clinical, and administrative support to clients through individual and group services within an integrated care model.

Responsibilities

• Assess client situations through interviews and history review
• Develop and execute individualized treatment plans
• Set milestones to evaluate client progress
• Communicate updates with relevant care teams
• Facilitate referrals to healthcare professionals and programs
• Perform crisis assessments and triage interventions
• Provide wellness-related and integrated care services
• Maintain accurate client documentation

Qualifications

• Master's degree
• Active New York LCSW license
• 1-2 years of clinical mental health experience
• Bilingual English/Spanish Mandate
