W2 Remote Patient Care Coordinator (RN)
Location: Remote (New York license required)

Compensation: $60,000 - $70,000 for Full-Time Roles
We are a leading, patient-centered, multi-specialty medical practice
in New York City. As part of our continued growth and
commitment to expanding access to care, we are launching telemedicine services
throughout New York to deliver high-quality gynecologic care to patients statewide. We
are seeking W2 Remote Patient Care Coordinators to support both our in-person
locations as well as our telemedicine expansion.
Position Summary
The Patient Care Coordinator is a licensed registered nurse (RN) responsible for
coordinating comprehensive, patient-centered care across both brick-and-mortar and
telemedicine platforms. This role serves as a central point of coordination between
patients, providers, and care teams, helping deliver a seamless, high-quality care
experience.
Key Responsibilities
Serve as a clinical liaison for patients.
Perform clinical intake, triage, and ongoing patient assessments within scope of
practice.
Collaborate with physicians, advanced practice providers, and interdisciplinary
teams to develop and implement care plans.
Provide patient education regarding conditions, treatments, medications, and
follow-up care.
Track and manage test results, referrals, prescriptions, and care transitions.
Identify and escalate urgent or complex clinical issues appropriately.
Support informed consent processes and ensure compliance with regulatory and
organizational standards.
Assist patients in navigating care pathways, including specialty referrals and
external services when needed.
Ensure accurate, timely, and compliant documentation in the electronic medical
record (EMR).
Monitor care gaps and proactively engage patients to support adherence and
outcomes.
Participate in quality improvement initiatives and cross-functional workflow
optimization.
Qualifications
Active, unrestricted Registered Nurse (RN) license in NY (multi-state licensure
preferred).
Minimum 2-3 years of clinical experience in ambulatory care, women's health,
telemedicine, or care coordination.
Strong clinical triage and assessment skills across diverse patient populations.
Experience working in or supporting multiple specialties preferred.
Proficiency with EMR systems.
Excellent communication, organizational, and patient engagement skills.
Ability to work independently and manage a high-volume, fast-paced virtual
environment.
Meticulous attention to detail.
Preferred Qualifications
Experience in multispecialty or integrated care settings.
Background in care coordination, case management, or population health.
Familiarity with telemedicine workflows, regulatory standards, and documentation
requirements.
