We are seeking a Social Care Navigator to help individuals access community resources, benefits, and services that improve health outcomes and support independence.Position Overview:At HDSW the Social Care Navigator (SCN) works directly with individuals to identify needs, and social support that promote stability, wellness, and independence. This role bridges the gap between clients and the complex social service systems '- Key Responsibilities:Conduct person-centered, strengths-based assessments to identify needs related to housing, food access, nutrition and transportationFacilitate referrals and warm handoffs to partnering agenciesEnsure continuity of care and follow up on referralsTrack client progress and document servicesEducate clients about available benefits and rightsComplete all documentation in accordance with OMH, DOH, HUD and HDSW standards.Track outcomes and service utilization for reporting Education & Experience Requirements: Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Public Health, or a related field with 2+ years of experience in case management, peer support, care coordination, or social service navigation OR associate degree with 4+ years of experience in case management, peer support, care coordination, or social service navigation Required Skills and Qualifications: Valid driver's license required Bi-lingual English and Spanish a plusNYS Justice Center Criminal History Background Check clearance requiredNYS Office of Children and Family Services for instances of child abuse and/or neglect clearance required.Must be legally eligible to work in the United States.
