A well-established Skilled Nursing and Rehabilitation facility located in the Brighton, NY area is seeking a dedicated and compassionate Social Worker to join our team.


This role is ideal for someone who is passionate about enhancing the quality of life for older adults and thrives in a supportive, interdisciplinary healthcare environment. The Social Worker plays a vital role in ensuring residents receive the emotional, social, and psychosocial support necessary to promote dignity, independence, and overall well-being.

Responsibilities include:
• Assessing and supporting residents’ social, emotional, and daily living needs to help maintain or improve their psychosocial functioning.
• Identifying individual concerns through ongoing interaction and documentation.
• Developing, implementing, and regularly updating individualized care plans tailored to each resident’s unique needs.
• Participating in admission planning, discharge planning, and community re-entry processes to ensure smooth transitions and continuity of care.

• Serving as an advocate for residents and families, providing guidance, resources, and support throughout their stay.
• Participating in interdisciplinary care plan meetings, collaborating with nursing, therapy, and administrative teams to ensure residents receive comprehensive, person-centered treatment.
• Maintaining strong communication with families, outside agencies, and internal departments.
• Ensuring compliance with all state, federal, and facility regulations related to resident rights and psychosocial care.

Requirements:
• Bachelor’s Degree in Human Services, Psychology, Social Work, or a related field.
• Prior experience in a long-term care or subacute rehabilitation setting is required.
• Strong interpersonal, communication, and organizational skills.
• Ability to work collaboratively as part of a multidisciplinary team.
• Compassionately serve a diverse resident population with professionalism and empathy.


Compensation:

$60,000-$65,000/year, and our generous benefits package!
