Encompass Health is seeking a Clinical Rehab Liaison to coordinate and manage inpatient rehabilitation referrals and admissions. In this role, you will conduct clinical assessments, collaborate with physicians, case managers, and payers, and guide patients and families through the rehab admission process. You’ll ensure appropriate level of care, secure insurance authorizations, and support smooth transitions from acute care to inpatient rehab. Working in a patient-centered, team-oriented environment, you’ll contribute to clinical excellence while growing professionally through specialty training and leadership opportunities. Coordinate and manage inpatient rehab referrals and admissions Conduct clinical assessments to determine rehab appropriateness and level of care Collaborate with physicians, case managers, and payers on patient placement Obtain and document insurance authorizations for rehab services Educate patients and families on inpatient rehab programs and expectations Facilitate safe, timely transitions from acute care to rehab Document assessments, referrals, and decisions in EMR and tracking systems Partner with the interdisciplinary team to support care plans and outcomes Build relationships with referral sources to support census growth Maintain compliance with regulatory, clinical, and documentation standards Clinical assessment Discharge planning Care coordination Rehabilitation services knowledge Insurance authorization Referral management Interdisciplinary collaboration Patient and family education EMR documentation Communication skills Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current RN, PT, OT, SLP, or related clinical license (preferred) Experience in acute care or inpatient rehab setting Knowledge of Medicare and insurance authorization processes Familiarity with EMR and referral management systems5c143e31-5e48-4549-b638-05792d185386
