Amergis seeks a mission-driven Dietitian to provide medical nutrition therapy across school, home, and healthcare settings. You will assess nutritional needs, develop evidence-based care plans, counsel patients and families, and collaborate with nurses, therapists, and physicians to support chronic disease management, pediatric and adult care, and wellness programs. Role includes documentation to regulatory standards, participation in interdisciplinary teams, and educating staff and communities while upholding Amergis’ high standards of clinical excellence and compliance. Conduct comprehensive nutritional assessments for diverse patient populations across school, home, and facility-based settings. Develop and implement individualized, evidence-based nutrition care plans for acute and chronic conditions. Provide counseling and education to patients, families, and caregivers to support behavior change and health goals. Collaborate with nurses, physicians, therapists, and school staff to coordinate interdisciplinary care. Document assessments, interventions, and outcomes in electronic health records in alignment with regulatory and payer standards. Support chronic disease management programs, including diabetes, cardiovascular, and weight management initiatives. Educate staff and community partners on nutrition best practices, special diets, and feeding protocols. Ensure compliance with federal, state, and organizational policies related to nutrition care and food service. Participate in quality improvement and outcomes tracking related to nutrition services. Maintain professional competencies through continuing education and adherence to clinical practice guidelines.
