Encompass Health is seeking a compassionate Physical Therapist to join our Healthcare & Medical Services team. In this inpatient rehabilitation role, you will evaluate and treat patients with diverse musculoskeletal, neurological, and post-surgical conditions, developing evidence-based, individualized care plans that promote optimal function and independence. You’ll collaborate closely with an interdisciplinary team, educate patients and families, and document outcomes in our EMR. Perform comprehensive physical therapy evaluations and develop individualized treatment plans. Provide evidence-based therapeutic interventions to improve mobility, strength, balance, and functional independence. Collaborate with physicians, nurses, and other rehab professionals to coordinate patient-centered care plans. Educate patients and caregivers on home exercise programs, safety, and ongoing rehabilitation needs. Accurately document assessments, treatments, and progress in the electronic medical record (EMR). Participate in interdisciplinary team meetings and contribute to discharge planning. Monitor patient response to therapy and adjust treatment plans based on clinical outcomes. Ensure safe use of therapy equipment and adherence to infection control and safety protocols. Provide guidance and mentorship to PT assistants and therapy support staff as appropriate. Engage in continuing education and quality improvement initiatives to enhance clinical practice.
