Patricia Neal Rehabilitation Hospital seeks a Dietitian to provide evidence-based nutritional assessment, counseling, and care planning for adult inpatients recovering from stroke, brain and spinal cord injury, and other complex conditions. You will collaborate closely with physicians, nurses, therapists, and case managers to optimize healing, manage comorbidities, and support functional gains. Responsibilities include nutrition screening, individualized care plans, documentation, patient and family education, and participation in interdisciplinary team meetings within a patient-centered, safety-focused culture. Conduct comprehensive nutrition assessments and ongoing reassessments for inpatient rehabilitation patients. Develop and implement individualized nutrition care plans aligned with medical and functional goals. Collaborate with physicians, nurses, therapists, and case managers in interdisciplinary team meetings. Provide patient and family education on diet, nutrition, and long-term health management. Monitor intake, tolerance, lab values, and clinical status to adjust nutrition interventions. Document assessments, plans, and outcomes accurately in the EMR and per regulatory standards. Support safe management of swallowing, enteral nutrition, and special diets in coordination with therapy teams. Contribute to quality improvement, safety initiatives, and best-practice protocols in nutrition care.
