Join Encompass Health and help every patient victory count. In this patient-facing rehabilitation role, you’ll assess needs, coordinate interdisciplinary care, and support recovery after major illnesses or injuries. Partner with physicians, therapists, and nurses to create and execute individualized treatment plans, track outcomes, and adjust interventions to help patients regain function and independence. You’ll document care in our EHR, educate patients and families, champion safety and quality standards, and participate in continuous improvement and professional development within a collaborative team culture. Conduct comprehensive patient assessments to identify rehab needs and goals Collaborate with an interdisciplinary team to develop individualized treatment plans Deliver and coordinate evidence-based rehabilitation interventions Monitor, document, and report patient progress in the EHR system Educate patients and families on recovery, safety, and home care strategies Ensure compliance with safety, quality, and regulatory standards Participate in case conferences and care coordination meetings Contribute to continuous improvement initiatives and best-practice adoption Support orientation and knowledge sharing with team members Pursue ongoing professional development and specialty training opportunities Patient assessment Interdisciplinary care planning Clinical documentation Rehabilitation techniques Care coordination Outcome measurement and tracking Electronic health records (EHR) use Patient and family education Safety and infection control Basic data analysis and reporting Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active clinical license appropriate to role (e.g., RN, PT, OT, SLP, or similar) BLS/CPR certification ACLS preferred depending on patient population Experience in inpatient rehabilitation or post-acute care preferred5c143e31-5e48-4549-b638-05792d185386
