Encompass Health seeks a compassionate Physical Therapist to deliver high-quality, patient-centered rehabilitation services in our inpatient rehab hospital. In this role, you will evaluate patients, develop and implement individualized treatment plans, and collaborate with an interdisciplinary team to restore function, mobility, and independence. You will provide therapeutic interventions, patient and family education, and thorough documentation to support optimal outcomes. Perform comprehensive PT evaluations and develop individualized treatment plans Provide therapeutic exercise, gait training, balance and neuromuscular re-education Educate patients and families on home programs, safety, and mobility strategies Document evaluations, progress notes, and discharge summaries in EMR accurately and timely Collaborate with physicians, nurses, and interdisciplinary rehab team on plan of care Participate in care conferences and contribute to discharge planning Monitor patient progress, modify treatments, and escalate concerns as needed Ensure compliance with regulatory, safety, and infection control standards Mentor or support therapy staff and students as assigned Contribute to quality improvement and evidence-based practice initiatives Physical therapy evaluation and treatment planning Therapeutic exercise prescription Gait and balance training Neuromuscular re-education Manual therapy techniques Inpatient rehabilitation clinical skills Patient and family education Electronic medical record (EMR) documentation Interdisciplinary team collaboration Knowledge of rehabilitation outcome measures Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active Physical Therapist (PT) license in practicing state Doctor of Physical Therapy (DPT) or equivalent accredited PT degree Basic Life Support (BLS) certification CPR certification Clinical experience in inpatient rehab or acute care Knowledge of Medicare and insurance documentation standards5c143e31-5e48-4549-b638-05792d185386
