Encompass Health seeks a dedicated Physical Therapist to deliver high-quality, patient-centered rehabilitation in our dynamic inpatient setting. You will evaluate functional limitations, design and implement individualized therapy plans, and collaborate with an interdisciplinary team to maximize patient independence and recovery. Responsibilities include direct treatment, patient and family education, thorough documentation, and participation in care conferences. Evaluate patients’ functional status, mobility, strength, and balance using standardized assessments Develop and implement individualized treatment plans focused on functional recovery and independence Deliver evidence-based therapeutic interventions, including gait training, therapeutic exercise, and neuromuscular re-education Educate patients and families on home exercise programs, safety, and adaptive strategies Collaborate with physicians, nurses, and the interdisciplinary rehab team on coordinated care plans Document evaluations, daily notes, and progress reports in the EMR accurately and timely Participate in care conferences and discharge planning to support optimal patient outcomes Monitor patient progress, modify treatment plans, and set measurable, achievable goals Ensure compliance with regulatory, safety, and infection-control standards Contribute to a positive, patient-focused culture and mentor less experienced staff as needed Physical therapy evaluation and treatment planning Gait training and balance rehabilitation Therapeutic exercise prescription Neuromuscular re-education techniques Pain management modalities (e.g., heat, ice, TENS) Use of assistive devices and adaptive equipment Inpatient rehabilitation care coordination Clinical documentation and EMR proficiency Patient and family education Interdisciplinary team collaboration Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active state Physical Therapist (PT) license Doctor of Physical Therapy (DPT) or equivalent accredited PT degree BLS/CPR certification Experience with electronic medical records (EMR) systems5c143e31-5e48-4549-b638-05792d185386
