Encompass Health seeks a compassionate Physical Therapist to deliver evidence-based rehabilitation services in our inpatient setting. You will assess functional status, develop individualized treatment plans, and provide hands-on therapeutic interventions to maximize mobility, independence, and quality of life. Collaborate closely with an interdisciplinary rehab team, educate patients and families, and document progress using EMR systems. Evaluate patients’ functional mobility, strength, balance, and pain levels Develop and implement individualized evidence-based treatment plans Provide therapeutic exercises, manual therapy, and gait training Educate patients and families on home exercise programs and safety Collaborate with an interdisciplinary rehab team on care plans and goals Document assessments, goals, and progress in the EMR accurately and timely Participate in discharge planning to support safe transitions of care Monitor patient response and adjust interventions as needed Adhere to regulatory, safety, and infection control standards Contribute to quality improvement and professional development activities Patient evaluation and functional assessment Treatment planning and goal setting Therapeutic exercise prescription Manual therapy techniques Gait and balance training Neuromuscular re-education Use of rehabilitation modalities (e.g., heat, e-stim) EMR documentation and clinical writing Interdisciplinary care coordination Patient and family education Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Current state Physical Therapist (PT) license Doctor of Physical Therapy (DPT) degree Basic Life Support (BLS) certification Experience with electronic medical records (EMR)5c143e31-5e48-4549-b638-05792d185386
