Encompass Health seeks a Rehab Physical Therapist to deliver evidence-based rehabilitation in our inpatient setting. You will evaluate patients, develop individualized treatment plans, and provide therapeutic interventions to maximize mobility, function, and independence. Collaborating with an interdisciplinary team, you’ll educate patients and families, track outcomes, and ensure safe discharge planning. Perform comprehensive PT evaluations and functional assessments for rehab patients. Develop and implement individualized, evidence-based treatment plans. Provide therapeutic exercises, gait training, balance and neuromuscular re-education. Collaborate with physicians, nurses, and other rehab team members on care plans. Educate patients and families on home exercise, safety, and functional goals. Document assessments, progress notes, and outcomes accurately and timely. Participate in discharge planning and recommendations for equipment or follow-up care. Maintain compliance with regulatory, safety, and quality standards. Contribute to performance improvement and patient satisfaction initiatives. Mentor or support PT assistants and students as appropriate.
