    • Provide patient and family education on rehabilitation goals and home exercise programsRequirements:
      • Valid and unrestricted Physical Therapist license
      • Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program
      • Minimum of 1 year experience working in a Skilled Nursing Facility (SNF) setting
      • Current BLS certification issued through American Heart Association (AHA)
      • Strong interpersonal and communication skills with commitment to patient-centered care
