The Medical Social Worker, under the supervision of the Clinical Manager, plays a vital role in the interdisciplinary care provided to patients receiving home health services. This position involves assessing and crafting individualized care plans that address the emotional and social needs of patients. Additionally, the Medical Social Worker will oversee and guide the Social Worker Assistant involved in the patient's care plan. Adherence to our organization's policies, healthcare standards, and relevant laws and regulations is essential.

Key Responsibilities
  • Evaluate the social and emotional well-being of patients in relation to their medical conditions, care requirements, and treatment responses.
  • Analyze the interplay between patients' medical and nursing needs, financial resources, and community support available in their home environment.
  • Implement strategies to access community resources that can assist in overcoming barriers to patient recovery.
  • Engage in communication with patients, their families, and other team members within the organization.
  • Continuously assess the needs of patients and families, aiding them in understanding the treatment process and managing emotional responses.
  • Collaborate as part of the interdisciplinary care team to coordinate care and serve as a resource for identifying and addressing patient needs.
  • Monitor the performance of the Social Work Assistant (BSW) to ensure compliance with organizational, governmental, and professional standards in delivering medical social services.
  • Maintain and submit necessary documentation, including case conferences, patient and physician interactions, visit reports, and progress notes, while collaborating with other healthcare professionals to ensure optimal patient care.
Essential Qualifications
  • A minimum of one year of experience as a social worker in a healthcare environment, including home health or hospice settings.
  • Licensure as a social worker in the state of practice, as mandated by state regulations.
  • A valid driver's license, auto insurance, and dependable transportation.
  • Current CPR certification.
  • Experience in discharge planning and accessing community resources (such as housing, shelter, legal assistance, and financial programs).
  • Knowledge of medications and their proper administration.
  • Proven ability to organize tasks, create action plans, prioritize effectively, and perform under pressure.
  • Willingness to work flexible hours and travel locally.
  • Maintain current licensure and fulfill mandatory continuing education requirements.
  • Fluency in reading, writing, and speaking English.
Additional Information

TB Screening: This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. Candidates selected for this role will be required to undergo TB screening.

Driving Requirements: This role is included in our driver safety program, necessitating a valid state driver's license and personal vehicle liability insurance. Individuals must maintain insurance that meets or exceeds state minimum requirements, or $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, or higher.

Work Schedule and Compensation

The estimated pay range for this full-time position (40 hours per week) is between $53,700 and $72,600 annually. This range reflects a good faith estimate at the time of posting and may vary based on geographic location, individual skills, experience, education, and certifications.
