Key Responsibilities- Conduct thorough evaluations of patients' physical abilities, including reviewing medical history, performance assessments, and setting measurable goals for rehabilitation.
- Communicate evaluation findings and treatment plans to patients and their families, actively involving them in goal-setting to promote adherence and motivation.
- Develop and implement personalized treatment protocols based on individual assessment findings, focusing on improving mobility, strength, and overall functional capacity.
- Collaborate with a multidisciplinary team of healthcare providers to ensure comprehensive care, recognizing potential changes in a patient's condition and adjusting treatment plans as necessary.
- Maintain accurate and timely documentation of patient care activities, including detailed encounter notes, interim progress reports, and discharge plans, aligned with regulatory requirements and best practices.
- Manage your schedule effectively to ensure timely patient care while meeting productivity expectations in line with department objectives.
- Participate in community outreach initiatives and marketing efforts to promote the rehabilitation center and its services.
- Engage in continuous professional development, seeking out training and educational opportunities to enhance clinical skills and knowledge.
- Act as a mentor to students and new staff, providing guidance and support in their professional journey.

The patients you will serve include a diverse population ranging from young athletes recovering from sports injuries to seniors rehabilitating after surgeries or managing chronic conditions. Qualifications- A Doctorate degree in Physical Therapy (DPT) is required.
- A valid Physical Therapist license in the state of Connecticut.
- Basic Life Support (BLS) certification through the American Heart Association is mandatory.
- Exceptional interpersonal and communication skills are essential for effective patient and family engagement.
- Strong organizational skills and attention to detail are required for maintaining accurate patient records and managing schedules.
- A minimum of 2 years of clinical experience in a physical therapy setting is preferred, although new graduates may be considered based on their educational background and internships.
- Demonstrated ability to work collaboratively in a team-oriented environment and to manage a varied caseload effectively.
