Work EnvironmentYou will work onsite in our rehabilitation facility, which is designed to promote healing and recovery through a warm and welcoming atmosphere. Our facility features private treatment rooms, well-equipped therapy spaces, and access to outdoor areas that encourage physical activity. You will collaborate with a well-rounded team of healthcare professionals, including occupational therapists, speech therapists, and nursing staff, to ensure the best possible outcomes for our patients.
Key Responsibilities- Conduct comprehensive physical evaluations that assess range of motion, strength, balance, gait patterns, and functional limitations for each resident.
- Create and implement individualized treatment plans based on thorough assessments, establishing specific goals, exercise routines, and using modalities such as heat therapy or manual therapy.
- Administer therapeutic exercises to improve strength, flexibility, balance, and coordination, focusing on effective pain management strategies tailored to individual needs.
- Guide residents in walking techniques and the use of assistive devices like canes and walkers while addressing fall prevention strategies in every session.
- Support residents in activities of daily living (ADLs) such as dressing, bathing, and transferring, ensuring they can perform these tasks independently or with minimal assistance.
- Educate residents and their caregivers on the proper techniques for exercises, home exercise programs, and strategies for fall prevention.
- Maintain meticulous patient records, documenting progress notes, treatment plans, and functional assessments to track improvements and adjust treatment plans as necessary.
Our primary patient population consists of older adults living within an assisted living context. Many of our residents face age-related challenges such as arthritis, osteoporosis, dementia, and Parkinson's disease. Your expertise will directly contribute to enhancing their physical capabilities and overall well-being, helping them to engage in physical activity and maintain their independence.
Qualifications- A valid Physical Therapy license in the state of New York.
- A Bachelor's Degree in Physical Therapy or a relevant field.
- Strong communication and interpersonal skills to build rapport with patients and their families.
- Ability to develop and implement patient-specific treatment plans that meet individual needs and goals.
- A genuine passion for helping patients achieve meaningful recovery and enhancing their quality of life.
- 2 years of clinical experience as a physical therapist, preferably in an assisted living or rehabilitation setting.
- Experience developing treatment plans that incorporate advanced therapeutic techniques and modalities.
