Work EnvironmentThe Physical Therapist position is an onsite role, primarily within patients' homes in the Meriden area. You will work closely with a dedicated team of healthcare professionals in a supportive and collaborative environment. Key ResponsibilitiesAs a Physical Therapist at Opti-Move Rehabilitation, your responsibilities will include:
- Conducting comprehensive evaluations of physical function and mobility for clients in their homes.
- Developing and implementing individualized treatment plans that address specific patient needs, ensuring achievable goals.
- Regularly reassessing patient progress through documented clinical observations to modify treatment plans as necessary.
- Completing all required documentation in a timely manner, which includes SOAP notes, progress summaries, and care plans using our electronic medical record system.
- Educating patients and their families on the recovery process and potential challenges, empowering them with knowledge and resources.
- Meeting minimum utilization goals and other business objectives aimed at enhancing our service delivery.
- Collaborating effectively with team members from various disciplines within Opti-Move to provide comprehensive care for clients who utilize multiple services.
- Championing the company's core values and embodying the organizational culture as an ambassador for Opti-Move Rehabilitation.
Our clientele comprises individuals of all ages, races, and backgrounds with diverse physical therapy needs. QualificationsTo be considered for this role, candidates must meet the following qualifications:
- A valid Master's Degree or Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program.
- Current and unrestricted licensure by the Connecticut Board of Physical Therapy.
- Flexibility to conduct evaluations and therapy sessions both in-person and via telehealth as clinically appropriate.
- A strong passion for working with a diverse patient population.

While not mandatory, the following experiences are preferred:
- Previous experience in home health care or outpatient settings.
