Job Description

Elaria Allergy & Integrative Health is a personalized, direct-care allergy practice focused on helping children and adults with food allergies, eczema, environmental allergies, eosinophilic esophagitis, and other complex allergic conditions.

We are looking for a motivated, compassionate, and highly organized Patient Care Coordinator to join our growing practice.

This role is 10 hours a week and consistent 1 hour involvement M-F. Virtual and Hybrid options available.

This is an excellent opportunity for more experienced with patient care coordination that want to have a part time healthcare related job, a pre-medical student, pre-health student, gap-year student, or healthcare professional who wants meaningful experience in a physician-led specialty practice and enjoys building relationships with patients and families.


The Role:


The Patient Care Coordinator will help ensure that patients have a seamless experience from scheduling through ongoing treatment. A significant part of the role will involve coordinating care for patients participating in food allergy immunotherapy/desensitization programs and other longitudinal allergy programs.


Responsibilities
  • Coordinate patient scheduling, follow-up visits, and treatment appointments
  • Communicate with patients and families by phone, text, email, and patient portal
  • Help patients understand next steps in their care plans and keep them on track
  • Coordinate ongoing care for patients participating in food immunotherapy/desensitization programs
  • Assist with treatment scheduling, follow-up, and routine patient communication
  • Track patient needs, appointments, forms, testing, and treatment milestones
  • Communicate with the physician and other members of the care team to ensure continuity of care
  • Help coordinate referrals, laboratory testing, outside records, and other aspects of patient care
  • Provide a warm, professional, and highly personalized patient experience
  • Assist with administrative and practice-support tasks as needed
  • Maintain patient confidentiality and follow HIPAA and practice policies

Qualifications

  • Professional phone etiquette and comfort handling high call volumes with empathy and clarity.
  • Experience with care coordination, including follow-up visits and referrals.
  • Basic understanding of medical terminology or willingness to learn quickly.
  • Prior experience in patient care, customer service, or a healthcare front office setting.
  • Proficiency with electronic scheduling systems and basic computer applications.
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to work reliably in a hybrid environment, maintaining confidentiality and HIPAA compliance.
